Participant Agreement, Code of Conduct & Liability Waiver
Event Participation Agreement I, the undersigned participant, voluntarily choose to participate in VOX Retreat, organized by Zion Church.
I understand that VOX Retreat is a Christian retreat intended to provide an environment for worship, teaching, prayer, fellowship, community, spiritual growth, recreation, and other retreat-related activities.
By signing this agreement, I acknowledge that participation in the retreat is voluntary and agree to abide by the expectations, policies, and instructions established by Zion Church and the VOX Retreat leadership team for the duration of the retreat.
Assumption of Risk I understand that participation in a retreat and its recreational, group, travel, and other activities may involve inherent risks, including but not limited to slips, falls, collisions, strains, sprains, cuts, illness, allergic reactions, or other injuries that may occur despite reasonable safety precautions.
I voluntarily assume the ordinary and inherent risks associated with my participation in VOX Retreat and its activities.
Release of Liability To the fullest extent permitted by applicable law, I release and hold harmless Zion Church, its pastors, leaders, volunteers, organizers, staff members, facilities, property owners, and affiliated organizations from liability for injury, illness, accident, loss, theft, or damage arising from my participation in VOX Retreat, except in cases of gross negligence, willful misconduct, or as otherwise prohibited by law.
I understand that I am responsible for my personal belongings throughout the retreat.
I have read and agree to the Participation Agreement, Assumption of Risk and Release of Liability. Medical Authorization In the event of an illness, injury, or medical emergency, I authorize VOX Retreat leaders or designated volunteers to contact emergency medical services and assist in obtaining appropriate medical treatment when reasonably necessary.
I understand that reasonable efforts will be made to contact my emergency contact when circumstances allow.
I accept responsibility for medical expenses incurred as a result of treatment provided to me.
By signing this waiver, I confirm that the information above is accurate and complete to the best of my knowledge and reflects the allergies I have disclosed to the VOX Retreat organizers. I am responsible for notifying the organizers of any changes before or during the retreat. I understand that allergen-free meals and an environment free from cross-contact cannot be guaranteed. I am responsible for checking ingredients, asking questions, and avoiding anything that may trigger an allergic reaction. To the extent permitted by law, the organizers are not responsible for reactions resulting from allergies I did not disclose or from my voluntary consumption of something I know contains an allergen affecting me.
I have read and agree to the Medical Authorization and confirm I disclosed my allergies. Retreat Code of Conduct I understand that VOX Retreat is a Christian ministry environment. I agree to conduct myself in a manner that respects the purpose of the retreat, the retreat property, leaders, volunteers, and other participants. I specifically understand and agree to the following:
1. Curfew — I will follow all established curfew and lights-out times communicated by VOX Retreat leadership. After curfew, I will remain in my designated sleeping area unless there is an emergency, medical need, or I have received permission from a designated retreat leader.
2. Sleeping Areas & Male/Female Boundaries — I will respect all designated sleeping and lodging areas. Participants may not enter sleeping rooms, cabins, or other designated private lodging areas assigned to participants of the opposite sex. I understand that these boundaries remain in effect throughout the retreat, including during free time and after curfew. Exceptions may only be made by retreat leadership for an emergency, operational necessity, or another specifically authorized reason.
3. Sexual Conduct — VOX Retreat is intended to maintain a safe environment consistent with the Christian values and standards of Zion Church. Sexually inappropriate conduct, unwanted physical contact, sexual harassment, sharing or displaying sexually explicit material, or behavior that compromises another participant's safety, privacy, or dignity will not be tolerated. Participants are expected to respect appropriate physical and relational boundaries throughout the retreat.
4. Remaining on Retreat Property & Personal Vehicles — Once checked in, participants are expected to remain at the retreat location until the official conclusion of VOX Retreat. Participants may not leave the property for personal errands, food runs, restaurants, stores, entertainment, or other non-retreat activities. Participants who arrive in personal vehicles agree not to drive or use their vehicle during the retreat. Leaving the property or using a vehicle is permitted only with authorization from VOX Retreat leadership or in the event of an emergency or exceptional circumstance.
5. Alcohol, Drugs, Smoking & Vaping — The possession, consumption, distribution, or use of alcohol, illegal drugs, marijuana, or other unauthorized intoxicating substances is prohibited during VOX Retreat. Smoking, vaping, and the use of tobacco or nicotine products are also prohibited during the retreat. Prescription and over-the-counter medications should be used responsibly and only by the person for whom they are intended.
6. Weapons & Dangerous Items — Weapons, firearms, explosives, and other items reasonably considered dangerous are prohibited from VOX Retreat except where otherwise required by applicable law.
7. Respect for Others — I agree to treat participants, pastors, leaders, volunteers, facility staff, and others with respect. Bullying, harassment, intimidation, threats, fighting, hazing, discriminatory harassment, unwanted physical contact, or behavior that places another person at risk is prohibited. I will respect the privacy, personal belongings, personal space, and boundaries of other participants.
8. Respect for Property — I agree to respect the retreat facility, the venue, leaders, volunteers, and other participants. I understand that I may be held responsible, to the extent permitted by law, for damage that I intentionally cause or that results from reckless conduct.
9. Following Leadership & Safety Instructions — I agree to follow reasonable instructions given by VOX Retreat pastors, leaders, staff, and designated volunteers concerning safety, scheduling, transportation, lodging, activities, and retreat operations. I understand that retreat rules continue to apply during free time and overnight hours.
10. Leaving the Retreat Early — If I need to leave VOX Retreat before its official conclusion, I agree to notify and check out with a designated retreat leader before leaving the property. I understand that I may not simply leave the retreat without informing leadership.
Violation of Retreat Policies I understand that failure to follow this Code of Conduct may result in consequences determined by VOX Retreat leadership based on the circumstances and seriousness of the violation. These consequences may include:
A conversation or warning from retreat leadership; Removal from a particular activity; Removal from VOX Retreat and a requirement to arrange transportation home. Participant Acknowledgment By signing below, I confirm that:
I have carefully read this Participant Agreement, Code of Conduct & Liability Waiver. I understand the expectations for participating in VOX Retreat, including the rules regarding curfew, sleeping areas, sexual conduct, remaining on the retreat property, driving, alcohol and drugs, respectful behavior, and following leadership instructions. I understand that although I am an adult and am participating voluntarily, participation in VOX Retreat is conditioned upon my agreement to follow these policies for the duration of the retreat. I understand that violating these policies may result in my removal from the retreat. I voluntarily agree to these terms and accept responsibility for my conduct while participating in VOX Retreat. I confirm everything in the Participant Acknowledgment and voluntarily agree to these terms.
